Swann Special Care Center believes in the importance of teamwork. We employ approximately 140 in-house personnel, contracts with various consultants and coordinates a volunteer network to provide the best possible care.
A wide variety of professionals are needed to ensure that the quality of life of each individual served is the highest possible. These professionals make up the interdisciplinary TEAM. The team concept is vital to Swann - without it, Swann could not do its work.
Circle of Support
It takes everyone working together to make certain that our residents receive the best of care. There are no "un-important" jobs. Every person employed is a member of the TEAM. Each person on the TEAM contributes to the individual's "Circle of Support."
Maury K. Topolosky, MD, FCCP Medical Director
Dr. Topolosky has served as the Medical Director at Swann Special Care Center since April 2011 and was named the Exceptional Living Centers Medical Director of the Year in 2013. He attended medical school at the Ohio State University College of Medicine. His surgical residency was completed at Washington University Barnes Hospital in Missouri and his medical internship was completed at Duke University Medical Center in North Carolina.
Dr. Topolosky is licensed in Ohio, North Carolina, Alabama and Illinois. He is a diplomat of the American Board of Internal Medicine and has served as the Clinical Assistant Professor of Medicine at the University of Illinois College of Medicine since 1985. Additionally, Dr. Topolosky is the Medical Director of the Respiratory Therapy Curriculum at Parkland Community College, the Medical Director for the Hospice Program at Presence Covenant Medical Center and the Department Head of the Division of Pulmonary Medicine, Critical Care and Sleep Medicine at Christie Clinic. From July 1974 to July 1975, Dr. Topolosky was aboard the USS Trenton with the United States Navy where he served as the Medical Officer. After this, he went on to become an Emergency Room Physician at Admiral Joel T. Boone Clinic in the Portsmouth Naval Regional Medical Center.
Dr. Topolosky is a member of the American College of Physicians and the American Thoracic Society. He is a fellow with the American College of Chest Physicians. He is on the Health Outcome and Peer Review Committee and Chairman of the Benefits Retirement Committee. Dr. Topolosky is ACLS Certified and ACLS Instructor Certified. Dr. Topolosky has also received the Physician Recognition Award from the American Medical Association.
Kym Halberstadt Executive Director
Kym has served as the Executive Director at Swann Special Care Center since 2009 and has been worked in long-term care for 25 years. She chose to work in a long-term care because she wanted to make a difference in someone's life. And, at Swann Special Care Center she fell in love with the great residents. Her primary responsibilities include ensuring the safety and well being of the residents and staff. Kym attended Parkland College and is a Certified Nursing Home Administrator. In her free time, Kym enjoys traveling with her husband, bike riding, gardening and reading.
Janie Breeze Director of Nursing
Janie has served as the Director of Nursing at Swann Special Care Center since 2010 and has been worked in long-term care for more than 13 years. She is responsible for managing the nursing department, scheduling, budget management and survey management. Janie earned her RN/BSN degree from Millikin University and became a staff nurse at St. Mary's Hospital. After retiring from St.Mary's, Janie chose to work in a skilled nursing center because of how rewarding it is. In her free time, she enjoys gardening, bowling and spending time with her family.
